Narrative Report

This is the primary report used by providers. It includes all client responses to ASI-Connect interview questions in a narrative format. Data are presented in each of the domains and clinicians are prompted to add information and clinical impressions.

Client Placement Worksheet (ASAM)

This worksheet is populated with data from ASI-MV Connect interview within each of the six dimensions of the American Society of Addiction Medicine (ASAM) Patient Placement Criteria. The clinician is prompted to add further information and rate the “Risk” of each dimension. This tool is intended to assist clinicians with effective client placement.

This report summarizes client self-reported data from across all domains, and arranges it in a useful manner to assist clinicians with treatment planning.

Sexual Assualt Summary

This report is a single-page summary of elements of a client's responses to the ASI-MV Connect interview that may be useful indicators for further assessment of whether or not the client is a victim of domestic violence and/or sexual assault.

Brief Screening Summary

This single-page summary provides key elements of a client’s responses to the ASI-MV Connect interview, including answers that may indicate psychiatric risk. It can be used to assist staff in deciding whether or not to refer a client for a full evaluation, as well as to assist in evaluating a client's immediate safety.
